In daily pathology laboratory operations, medical and technical staff frequently handle unfixed tissue specimens, blood, body fluids, and various volatile chemical reagents. These materials may contain potential pathogens such as HBV, HCV, HIV, and Mycobacterium tuberculosis, as well as unknown infectious agents.
At the same time, pathology work involves exposure to hazardous chemicals such as formaldehyde and xylene.
For these reasons, the Biological Safety Cabinet (BSC) is an essential piece of equipment in pathology laboratories, forming a critical barrier that protects personnel health and ensures diagnostic quality.
1. Biological Hazards
Sources: Fresh surgical specimens, frozen section samples, body fluids, blood specimens, and cytology or fine-needle aspiration samples
Risks: Aerosol generation, splashing, or accidental exposure during cutting, grossing, and specimen handling
2. Chemical Hazards
Sources: Volatile reagents such as formaldehyde (formalin), xylene, and ethanol
Risks: Formaldehyde is a confirmed carcinogen, and long-term exposure poses significant health risks
➡️ Standard clean benches or chemical fume hoods cannot provide simultaneous protection against biological and chemical hazards. Only properly selected biological safety cabinets can meet the specific requirements of pathology laboratories.
Class II Type B2 Biological Safety Cabinet (Total Exhaust Type) is the preferred and recommended solution.
Operating Principle
All air entering the cabinet is 100% HEPA-filtered and exhausted directly to the outside
No air is recirculated within the cabinet, preventing the buildup or return of contaminants
Key Advantages
Superior chemical protection: Volatile chemicals such as formaldehyde and xylene are fully exhausted, preventing accumulation in the cabinet or laboratory
Reliable biological protection: Provides personnel, product, and environmental protection in compliance with biosafety standards
Typical Applications
Grossing of surgical pathology specimens
Frozen section sample handling
Cytology and liquid-based cytology (TCT/LBC) processing
Important Notes
❌ Class II A2 cabinets are not recommended for operations involving volatile toxic chemicals due to partial air recirculation
❌ Laminar flow clean benches are strictly prohibited for pathology operations with biological or chemical hazards, as airflow is directed toward the operator
Gross Specimen Dissection
All fresh surgical specimens must be measured, described, dissected, and sampled within a Class II Type B2 cabinet.
Frozen Section Procedures
Due to the high-risk nature of fresh tissues, specimen trimming and transfer steps should be performed inside or adjacent to the safety cabinet.
Cytology Sample Processing
Includes handling of pleural effusions, ascites, sputum, and fine-needle aspiration samples for slide preparation.
Pre-Analytical Procedures for Special Tests
Sample preparation prior to microbiological culture, PCR, or other molecular analyses.
Personal Protective Equipment (PPE): Laboratory coats, waterproof aprons, double gloves, and eye or face protection are mandatory
Proper Workspace Organization: Separate clean, working, and contaminated areas; never block front or rear airflow grilles
Safe Operation: Use slow, controlled movements to minimize turbulence and aerosol generation; dispose of waste as regulated medical waste
Cleaning and Disinfection: Thoroughly disinfect cabinet surfaces daily using compatible agents such as 70% ethanol
Certification and Maintenance: Annual professional certification is required to verify airflow velocity and HEPA filter integrity
In pathology laboratories, a biological safety cabinet is not an optional “advanced workstation" but a fundamental component of the biosafety infrastructure.
Selecting and correctly using a Class II Type B2 biological safety cabinet demonstrates a strong commitment to personnel safety, laboratory quality management, and diagnostic reliability.
It serves as both a shield against biological and chemical risks and a cornerstone for ensuring that pathology diagnostics remain safe, accurate, and sustainable.
